We request personal details, including PHI (e.g., names, addresses, medical history, diagnoses, treatments, payment info), solely when essential for delivering our telemedicine services. This information is obtained through ethical and legal channels, with your awareness and express written consent where required under HIPAA, TDPSA, FDBR, and state telehealth laws.
Collected data is kept only for the duration required to achieve its original intent or as mandated by law (e.g., at least 6 years under HIPAA). We secure stored information using HIPAA-compliant protections, including encryption, access controls, and regular risk assessments, against potential loss, unauthorized entry, exposure, duplication, misuse, or alterations. All electronic health records are physically maintained in the United States, in compliance with Texas and Florida data localization laws.

Uses and Disclosures of PHI

We use PHI for treatment (e.g., sharing with pharmacies), payment (e.g., billing), and healthcare operations (e.g., quality improvement). Disclosures may occur to business associates under agreements, or as required by law (e.g., public health reporting). We obtain authorization for other uses, such as marketing.

Text Message Communications and Agreement

Altius Performance Medicine could dispatch text messages to those who have chosen to receive them. Such texts might encompass, among others:

Links for telemedicine visits
Invitations for downloading HIPAA compliant messaging apps
Questions about billing
Special promotions or deals (where relevant)
These communications comply with TCPA and HIPAA; no PHI is sent via unsecured SMS unless you consent.

Frequency of Messages

The rate of messages can differ by communication type, with a maximum of up to 5 SMS texts weekly for topics like scheduling, payments, or offers.

Methods for Opting In and Out

To opt in for text messages, users may:
Give verbal agreement
Fill out a digital form
Sign a physical document
Opt-in constitutes express consent under TCPA for non-marketing texts.
Opting out is possible anytime by texting “STOP” back to a received message. Users can also reach out to us to be taken off the list.

General Texting Notices

Standard rates for messages and data might be charged by your carrier.
Reply “HELP” for support or check our Privacy Policy section.
Messaging rates can fluctuate.

Data Distribution

Altius Performance Medicine avoids sharing text opt-in details or contact numbers for SMS with external parties or related firms for promotional reasons. Our site could connect to outside websites beyond our operation. Note that we lack oversight of their materials or procedures and bear no accountability for their privacy approaches.

Disclaimer & Compliance Notice

Medical services are available only to patients physically located in Texas, Florida, or North Carolina during consultations, in accordance with 2025 state telemedicine regulations. Initial consultations may require synchronous video in FL and NC; asynchronous messaging is allowed in TX for established patients. All providers are licensed in the states they serve.

Prescriptions require a valid patient-provider relationship established via synchronous or asynchronous methods per state law.

Testosterone therapy is FDA-approved for clinical hypogonadism; compounded formulations are not FDA-approved and are prescribed under 503A for patient-specific needs. DEA compliance is required for testosterone prescribing.

Compounded GLP-1 medications (e.g., semaglutide, tirzepatide) are not FDA-approved and are used only under 503A regulations when FDA-approved options (e.g., Wegovy®, Zepbound®) are unsuitable due to documented intolerance or unavailability; compounded versions must be clearly labeled with risks, per 2025 FDA guidelines.

All benefits mentioned (e.g., for testosterone, weight loss, hormone optimization) may include improved energy, weight, mood, or libido, but results vary by individual and are not guaranteed, per FDA/FTC guidance.
